Join #KareoChat for Latest on Chronic Care Management

October 27th, 2015 by Kareo

Chronic diseases are among the most common, expensive, and preventable health problems in the United States. In fact, 86% of healthcare spend goes to treat chronic disease, and about half of the adults in America, approximately 117 million people, have at least one chronic condition. Research consistently shows that effective care coordination between medical visits reduces the costs of care for chronic disease patients but improves their overall health. Recognizing this, Medicare is now reimbursing providers who deliver between-visit care to their eligible Medicare patients.  Chronic Care Management (CCM), CPT code 99490, pays an average of $43/patient/month. However, practices have struggled to implement this new program. Join @GoKareo and @CareSync on Thursday, October 29 at 9:00 AM PT for #KareoChat to discuss the latest updates on CCM and what can be done to make CCM more broadly utilized.

Here are the chat questions to help you prepare your thoughts and join the conversation.

  1. What have been the biggest changes we’ve seen during this first year of #CCM?
  2. What are the obstacles that keep providers from CCM?
  3. What would make providers more likely to implement a #CCM program?
  4. What do patients expect from CCM?
  5. Has CCM been what you expected when it was announced at the beginning of 2015?
  6. If you could change/improve the program what would you do?

This is a great chance to gain new insights and share your perspective. Hope to see you at #KareoChat!
